Our teams will help by discharging patients safely as well as providing ongoing support to foster independence, improve wellbeing and minimise the risks of early readmission.
The service is funded by Buckinghamshire Council and is in joint partnership with Age UK Buckinghamshire and Age UK Hillingdon Harrow & Brent.
This service supports patients being discharged from both Stoke Mandeville and Wexham Park Hospitals. Our friendly Discharge Team will transport people home and help them settle in after their hospital stay. Our Community Support Team can provide 4-6 weeks of support at home to help with shopping, light housework and emotional support in order to prevent readmission and improve independence at home.
Prevention matters is a free short term service that will help you build a more independent and fulfilling life. Make friends, feel safer, regain confidence, remain active, develop hobbies, improve your health, be part of the community, attend local event, access a range of local support. – PLEASE BE AWARE, you are NOT eligible for this service if you have a care package in place.
