Arc Bucks PCN covers a population of around 85,000 patients making it one of the larger PCNs and so we have split our team across two hubs. One in Marlow and one in Beaconsfield.
Some of the different roles based at the BAT Hub include Care Coordinators, Social Prescribers, a Health and Wellbeing Coach and a Physician Associate. The wider PCN team also includes Pharmacists, Pharmacy Technicians and First Contact Practitioners offering Musculoskeletal services.
The BAT Hub is located in the Beaconsfield Medical Centre.
The Hub is open between 9am and 5pm, from Monday to Friday by the Care Coordinators. Incoming calls and emails can be answered and responded to during these core opening hours. The other members of the Access Team are based between the hubs, the practice sites and out in the community.
