Jo enjoys bringing her experience working as a GP to her role as clinical support with the PCN. Her passions are in mentoring, teaching, training, elderly care and working with care homes. She is inspired by having enthusiastic and dedicated colleagues to work with to provide a high standard of patient care in the local area.
Patient care and safety is of the upmost importance for Lynette, who has a methodical and person-centred approach to mental health support care. This involves building trust and confidence between practitioner and patient, problem-solving, risk assessment and follow-ups to aid in deciding the most appropriate, individual care plans.
With a background in the Ambulance service, John has brought with him the passion of service, sense of duty, directly saving lives and providing support and comfort to those most vulnerable in the community. John sees patients in the GP surgery, care homes and also provides home visits to housebound patients.
Liam responsibilities as a PCN paramedic has evolved over time; now managing a wide range of medical issues, including minor illnesses, long-term health reviews, and urgent care needs. Liam enjoys the balancing of traditional paramedic skills with new competencies required in a GP environment, which in turn, has broaden his clinical expertise.
Mohammed believes that empathy is the first, and most important, step to beginning to help someone in need, and small impact can hugely help change someone’s wellbeing. Weekly care home visits and GP practice patients gives Mohammed a variety of patient-focused appointments and to provide a supportive service.
Sarah likes to see the small changes a patient can make when supported by a Social Prescriber and spring board them into obtaining goals that they didn’t think possible. Sarah enjoys supporting and managing the Social Prescribing Team, to see the service grow and become embedded in the Primary Care offer to the patients in Arc PCN.
After working in community home care, special needs schools and hospitals, Rosie has experience in a range of health care demographics. This is why she believes in promoting independency by being a positive and supportive to a patients needs. Seeing that progress is most important to Rosie.
Experience in working with mental health and learning disability patients has given Leeah first-hand experience helping those most vulnerable people in the community. Leeah’s satisfaction comes from knowing everyone is working towards the same goal; providing safe and confidential space for those to seek help.
Jackie’s motivations are found in knowing she is making a difference with the patients she works with. She achieves this by seeing the bigger picture in patient’s health and wellbeing; reinforcing and encouraging the importance of social interactions and seeking assistance.
Maxine is passionate about the world of preventative health care and the exciting field of diet and lifestyle ‘medicine’. Focusing on blending the holistic approach of Nutritional science with positive behaviour change.
With a strong commitment to evidence-based care, innovation, and collaboration, Sushma is driven to lead holistic initiatives for public health, and empowering individuals to achieve sustainable health outcomes that impact wellbeing.
Prianca aims to enhance the value and outcomes of medications by conducting thorough medication reviews and prioritising the needs of patients. She takes pride in collaborating closely with care homes, offering personalised advice and address medication-related issues using a holistic approach.
Samiha is driven by a deep commitment to patient centred care, with a passion for improving long-term health outcomes, focusing on polypharmacy, deprescribing, and evidence-based medicine. Her belief is that effective healthcare is rooted in a partnership between patients, healthcare providers, and the community.
Medication optimisation and being proactive is key to Emma’s philosophy as a pharmacy technician. She drives to ensure patient medication needs are met but also on looking at things more holistically when it comes to patient-centred care.
Kam’s passion is to provide the excellent person centred care for patients; helping with making small changes which will make a difference in people’s lives. Her previous background in retail and hospital settings has only helped to mould her vision and work within the PCN.
Kulpreet achieved honours in Occupational Health, Safety and Environmental Management, which has enhanced her approach as a Care Coordinator. She has a drive for continuous learning in order to make a positive impact on patient care and seamless operational processes.
Kim has helped foster a collaboration with multidisciplinary teams, and has a responsible commitment to coordinating appointments, managing care plans, and supporting individuals in long term care; improving health outcomes and enhancing the patient experience.
Rachel’s previous local community pharmacy experience brings a personal touch to our MAT care coordinator duo. Her enjoyment comes from working with the wider team and closely within her own role and direct colleagues.
His time working within the NHS, primarily specialising in Urgent & Same Day Care, Jake focuses on the patient, making sure that they are respected, listened to and their care needs meet appropriately, often using skills and knowledge to signpost and being someone who can support patients in difficult times.
With 6 GP practices and 2 access hubs, Jonathan drives for a strong and immediate communication access between the PCN network, patients and local community. Improving and modernising the understanding of digital services is how Jonathan strides to make a difference to people’s digital accessibility.
Working within the care sector for 20+ years, and more recently as a social prescriber elsewhere in primary care,Chris is passionate about helping patients to navigate the care system, to find support and activities for their needs within the local community.
