Clinical Pharmacist

Clinical Pharmacists are highly skilled experts in medicine and can help people in a number of ways. This includes providing support to manage long-term conditions, advice for those on multiple medicines and provide better access to health checks, such as for blood pressure. The role is pivotal to improving the quality of care and ensuring patient safety.

Many Clinical Pharmacists can prescribe medication independently, within their scope of competence and working in partnership with the GP Practices.

Clinical Pharmacists can support practices with a range of activities, including:

  • Reviewing your medication.
  • Making decisions and changes to your prescriptions.
  • Providing information and advice about the safe and effective use of your medication.
  • Identify patients who are at high risk of medication-related problems.
  • Works closely with local care homes as part of the Enhanced Health in Care Homes framework.
