Community Connectors

An integral part of the Arc Bucks PCN Model of Care involves the voluntary sector to enhance the Community and provide a network of interested individuals working towards common goals. The objectives of Arc Bucks PCN are to provide better access to support in community, a holistic model of care allowing joined up working across medical health, social support and overall wellbeing of patients within the area. Armed with signposting skills and a wealth of informaiton found here on our Directory of Services, everyone can find the support they need.

Why signpost?

  • There is a huge amount of support out there but lots of people don’t know about it.
  • We don’t want people to say ‘if only I had known that’ … we want people to say ‘I know where you can find out.’
  • The best people to pass on information about support aren’t necessarily ‘professionals’ but people who we know and trust, this helps engage with hard to reach people.
  • Gives people information early and empowers people with knowledge to prevent escalation.
